High Alert Medications


OASIS-C asks us if we have taught on high alert medications on admission and resumption of care.  The chapter 3 instructions state, “High-risk medications are those identified by quality organizations (Institute for Safe Medication Practices, JCAHO, etc.) as having considerable potential for causing significant patient harm when they are used erroneously.”

The link below is the list from the Institute of Safe Medication Practices.  It is the one we like best becaue it was originally designed for community pharmacies and doesn’t include medications only found in ICU’s and Operating rooms.  If you know of another list that might be worth taking a look at, please forward!

  1. We are using Beers list that our Med dir went over and signified the most used drugs and which ones could cause falls etc

    The list entitled “High Alert Medications” was made in 2008. Do you know if they are going to come up with a more current version.

    • Great question. That was the list on the ISMP website yesterday. I will email them and ensure that it is the most current list.

  3. That list still has way too many you will never see in the home. And remember, therapists are expected to do this teaching too! We went through and also modified the list to those that you are most likely to see in the home, that are most likely to cause problems by themselves, and we made a sheet the therapists (and nurses) can use with the most necessary first visit teaching.
