I received, via email this morning a copy of a study conducted by a company called Avalere Health, LLC documenting research on home health and the ability to save Medicare dollars by reducing hospitalizations in patients with chronic disease states such as failure, diabetes and COPD. This information needs to be shared and understood by all of us seeking to provide care for the post acute hospital patient. It further emphasizes the need for home health to work towards a significant reduction of hospitalizations for home health care patients.
